Communicare Health Centers
CommuniCare is both a Migrant Health Center and a Federally Qualified Health Center with multiple locations in Yolo County, California. As the largest Migrant Health Center in Yolo County, CommuniCare exists to help bridge the healthcare gap for this population. We provide a variety of culturally humble behavioral health services, including individual and group therapy, in English and Spanish. The Creando Recursos y Enlaces Para Oportunidades (CREO) program offers regular support groups facilitated by a promotora. CREO is tailored to meet the needs of the Spanish-speaking community and to educate them on stress management techniques, self-esteem, parenting, and other topics. CommuniCare is currently training a larger team of promotores who will expand our ability to outreach to the migrant community. Our team of Client Benefits Advocates, all of whom speak fluent Spanish and English, assist migrant and agricultural workers with enrollment in insurance and other public assistance programs such as CalFresh. CommuniCare’s Food Programs include classes that increase participants? access to healthy food, build food skills, and reduce social isolation. The quarter-acre Food is Medicine Garden provides a relaxing outdoor setting for implementing both food and mental health activities.

The goal of our Injury and Violence Prevention Program is to eliminate preventable death and disability due to injury and violence. We work to reduce injury risk in Washington State. We use our data to identify prevention priorities and develop interventions based on best practices.
