CommuniCare Health Centers

“CommuniCare is both a Migrant Health
Center and a Federally Qualified Health Center with multiple locations in Yolo County,
California.
As the largest Migrant Health Center in Yolo County, CommuniCare exists to help bridge the
healthcare gap for this population. We provide a variety of culturally humble behavioral health
services, including individual and group therapy, in English and Spanish. The Creando Recursos
y Enlaces Para Oportunidades (CREO) program offers regular support groups facilitated by a
promotora. CREO is tailored to meet the needs of the Spanish-speaking community and to
educate them on stress management techniques, self-esteem, parenting, and other topics.
CommuniCare is currently training a larger team of promotores who will expand our ability to
outreach to the migrant community. Our team of Client Benefits Advocates, all of whom speak
fluent Spanish and English, assist migrant and agricultural workers with enrollment in insurance
and other public assistance programs such as CalFresh. CommuniCare’s Food is Medicine
initiatives include classes that increase participants’ access to healthy food, build food skills, and
reduce social isolation. The quarter-acre Food is Medicine Garden provides a relaxing outdoor
setting for implementing both food and mental health activities. “

“For nearly 30 years, HEC has cultivated health and well-being in
under-served communities by leveraging the power of collaboration. For over a decade, HEC has administered
health programs at the Mexican Consulate in Sacramento including Ventanilla de Salud (VDS) which provides
free health-related services for immigrant and Latino clients.
A key program of VDS is Mente Sana, Vida Sana (Healthy Mind, Healthy Life) which reduces unmet mental
health needs in the Mexican immigrant community through PHQ9 depression screenings, referral and stigma
reduction. In the fall of 2017, HEC received funding from the Office of Health Equity to offer a community defined mental health initiative called the Mente Sana, Vida Sana (MSVS) program. MSVS addresses
awareness, early detection, and treatment of depression to improve individual and community well-being. “

Western Center for Agricultural Health and Safety (WCAHS)

“WCAHS is a multidisciplinary center located at the University of California, Davis. One of 11 agricultural health and safety centers in the U.S established by the Centers for Disease Control and Prevention (CDC) and the National Institute for Occupational Safety and Health (NIOSH), WCAHS has an almost 30-year history of engaging agricultural communities in the Western states of Arizona, California, Hawaii, and Nevada through innovative research and outreach.

WCAHS faculty and staff conduct research on high hazard topics, including air quality, ergonomics, dairy pathogens, and heat illness. Through its robust outreach program, WCAHS reaches over 3,000 individuals annually with health and safety trainings, health fairs, and presentations.”
